As of May 18, 2010, the state of Nevada DMV now has a form no. DLD-136, Medical Certificate and Authorization for Gender Change.
An MD just has to sign it and check the boxes that you are living as a female (or male for ftm's) and your driver license or ID will reflect the preferred gender marker. This allows pre- and non- ops to have ID matching their life.
I am one of the very few that got it when I asked for it in 2007, with my name change.
This is the best news I have heard in a long time.
Colorado and California are two other states I know of that will do this. Feel free to add any others.
