Questions on name change...

Ok so as soon as I have the money for the court fees I'll be submitting my forms for a name change. Arkansas does not have an "official" form so I pulled a sample off the internet and typed up one in word... Apprently you can even handwrite the petition.

So... I was wondering when I turn the forms into the clerk what should I put them in? I was thinking a report folder.
