I hope I don't drive everyone crazy with my questions. But could you tell me the difference between breast insert and forms? Which one do you and would you use? Where is the best place to get them in your opinion?

As I understand it:

A breast insert is like implants, it is surgically inserted into your breast.
Forms are silicon breast that you simply slip into your bra or attach to your breast using an adhesive.

I think rickie means Bra inserts (please, correct me if I'm wrong here) which are mainly to enhance what you already have, while forms actually give you something.

I think you will find inserts are like chicken fillets, which are incerted in to a bra to create a bigger cleavage, or bust than the lady actually has, where as a form is a total replacement for either a lost breast, or to provide the shape of a whole breast

I think that sound the most logical, but other may have different understanding of the two!
