What is the most difficult nail for you to paint?

For the life of me I just can't seem to get the big toes painted without having to start over at least once. One bad stroke and it's start over time.

You can get foam forms that fit between your toes to hold them still and apart from one another. They do help. I hold my big toe just above the first joint with my free hand, usually works.

My right thumbnail.

I rarely do my toenails; and I do not have any difficulty in putting my food on my opposite thigh. (I'm not especially limber, but crossing my legs is something I've kept up since childhood.)

I have a hard enough time clipping my toenails, never tried painting them. But the one I have the most problems clipping (And thus likely painting) would be my baby toes on either foot.

I am not flexible at all, so I can't really cross my legs at all.

So why bother painting them? LOL.
