My first thought is "No, don't do it." You might get excited and THAT would be awkward.
I always wear loose-fitting athletic shorts for the dermatologist visits. I feel better about wearing only those in front of the doctor and the other person(s) who might be in the room.
BTW: I have gone to the dermatologist on several occasions while dressed pretty. (I present male and wear skirts, etc.) I just change into my athletic shorts (no underwear) for the exam where I am supposed to be under the big paper "napkin of shame".
I have also gone to my normal doctor a couple of times while dressed in a skirt. I stopped doing that because my blood pressure was always reading high for those visits. (Also, it was dumb to go there. It is too close to home for someone who is keeping their crossdressing a secret.)

I will add that both experiences (dermatologist and family doctor) were basically normal experiences for me.
I have been going to the same doctor for over 25 years. Since I restarted cross dressing, I have kept a couple pairs of men's underwear when I have an appointment. He is retiring at the end of the year so we need to find a new doctor. My wife wants a female this time and I have no problem with that. Since she will be new and never seen me in men's undies, I'm thinking of wearing panties from the start with her. I like the idea of plain white cotton ones, at least to start.
I totally understand your dilemma. I?ve given this a lot of thought, but am still on the fence. Yes, Dr?s have seen everything but they still talk. My dermatologist, a female, always has an assistant in the room. Bottom line, while hugely tempted, I hold off.
I would just wear whatever you wear on a daily basis. If you want to wear panties to be you there are 363 other days you could do that. If you want to show someone your feminine side that's more a thing for a therapist or trusted friend than a doctor. If you want a thrill of maybe being discoverd thats up to you if it excited you.
I don't see this as a problem. I have been going to my dermatologist ( female) for about 8 years and never had a comment about my underwear. I underdress everyday so it is just what I wear. I did wear a plain pair nothing with lace or bows. I have even gone in the past year with my toe nails painted. I had forgotten about my Dr. appointment and had just gone for a pedicure with color a few days before. I didn't want to have to redo my toes so I said to myself the heck with it. Not a word was said. I also wear panties to my annual physical but don't normally have to undress.
The thing that would concern me about going in any form of non-male dress is the level of care that I would receive. I mean, I don't think discrimination would happen, but we know from multiple other sources that discrimination in other forms definitely happens in the medical field. I don't need that stressing my mind.
This week, I had to see a doctor on relatively shorter notice than I was expecting. They gave me a same day appointment, or the following day. I chose the following day because at the time they wanted to see me same day I would not have had an opportunity to go home and get changed out of my underdress attire. Further, I didn't want them to see impressions of bra straps on me. As expected, I had to undress fully for the visit, so I'm glad I took the following day.
Plus; I imagine the same problem would happens to my blood pressure as jjjjohanne; I would get a fairly higher reading. My BP is good (not perfect, but pretty good). I don't need to bump it up into danger levels for an appointment.
Crossdressing in public is not my normal attire. In fact, I rarely dress in public. Going to the doctor dressed in anything femme would not feel right; I would feel like I was involving them in my adventures.
But, all of that is just me. Your mileage may vary :)

Do you want to do it because you underdress all the time and don't want that to change?
sure go for it, you're not really doing anything different than you would for anything else
Do you want to do it to elicit a thrill out of the dr. seeing you in them?
that feels too much like exhibitionism for me (which is something I'm personally very cautious of, and admittedly have overstepped once or twice, not that I'm proud of it).

The ONLY underwear I own, and wear, are lace hipsters in a variety of nice colors. I wear them 24/7, including doctor appointments. I also, if the weather is cool and dictates a light jacket or windbreaker, always wear a lace bralette. I've no idea if my panties have been seen, but I suspect they have. I know my bralette has been detected whenever a doctor, nurse, or tech has used a stethoscope to listen to my heartbeat. Nobody has ever said a thing. What could they say other than, "You're wearing a bra/panties?" The only answer can be, "Yes." It's a total non-issue and I am basically in the closet with regard to all family and friends (except for 2 trusted people). Go for it.
The key is that you're in the closet. Letting others see you in lingerie will open that closet door a bit.
I live 24/7 and have been to a variety of doctors and my dentist as a woman and have never had a big problem.
I am out to my (female) physician about having gender dysphoria. But I have yet to wear panties to a checkup, because 1)I want my medical visit to be about my medical condition and nothing else, and 2) like JJJJohane, I have a blood pressure issue, and don't want to do anything that would raise it in an exam.
If you are going to wear panties I'd suggest shaving off body hair for painless EKG tests!
I wear ONLY women's panties anymore. I have many different styles and colors but one thing is for sure; they are clearly feminine underwear. The male underwear is gone. I have even had three surgeries and been hospitalized twice in women's panties. There has never been a comment about them except from my wife.
Panties are underwear to me, worn for many years. 1st pair like at age 5 was a friends of mine. We switched underwear while playing one day. So basically 64 years of panties.
My many style is slip shorts, I worn black before.
Typically they leave the room for you to undress, for the exam.
Then come back in the room.
I have had a shot in the rear before wearing black slip shorts.
Would I wear colorful / lace to the doctor no.
Slip shorts have very few color choices.
I only have panties in the underwear drawer now. I have a couple pairs of Jockey briefs in black and navy that are hard to differentiate between panties and men?s underwear. They are my go-to for doctors visits! Only the discerning healthcare provider would notice the difference, which is a bit exhilarating to be honest!
It's been said but Doctors (of all genders) have seen it all and are just there to do what they have to do. I wore (and wear daily) panties to my last prostate exam. Doctor (M) did not bat an eye or say a word when I was asked to drop my trousers for the exam... no gown and he never left the room.
I've seen the same Primary Care for 20+ years. 5 years ago, I started wearing panties and the occasional bra to appointments. But, I don't wear a bra to my annual physical, as I know my lines will be obvious. I know her well enough that if we had a conversation all would be cool. And I don't think she would slip and say anything to my wife about it.
Last year, I went to a dermatologist for the first time. My Primary found a couple spots that looked odd, so she gave me a referral. I had a full body exam. I was wearing panties of course ( that's all I have), but wasn't sure about the new doctor. She's also a young 30-ish female.
I was told I could strip all the way or leave my underwear on. Since I wasn't feeling so certain, I took everything off.
And I felt very awkward being completely naked!
So, at my first follow-up visit, and every visit since then (every 3 months because I've had 2 melanomas), I leave the panties on. Somehow, I felt way more comfortable wearing panties in front of her than being completely naked. I don't understand it. I'm sure I'm not the first she's seen, and of course she doesn't say anything about it.
Now, going to the doctor is just something else I do underdressed every time.
I have spoken to my wife and other GG?s about what they wear and for them they just go basic panties. Not the sexy Lacey or thong etc. I have always gone drab but more recently I have some boyleg panties that aren?t super obvious. I have worn for my skin cancer checkup regularly and even my GP and specialist recently as I have a bit of a downstairs problem requiring attention and nothing said.
I went to a dermatologist appt wearing a pair of athetic branded bikinis (rebock i think). I'm not sure anyone who wasn't really interested couldn't tell they weren't just some nice men's underwear. My pink toenails were more of a distraction than my panties when the full body scan took place.
