Escapism isn't necessarily bad, but is definitely unhealthy in the long term. While helpful in the short term, things will degrade over time. At some point, the escapee will have to face the issue. Things simply blowing over isn't really going to happen in many situations.

Definitely number one is the weight. After that, there are so many other things I want to change, it's hard to choose... but, I guess number two would be to let my real hair grow out and then dye it to the color of my wigs. Then the third would be my voice - something I've been practicing, but still have a ways to go.
OK, this is cheating, but number four would be to get a real, permanent boob job.
#1 a big juicy butt. not too big obviously
#2 natural breasts
#3 smaller hands & feet.
1. Lose weight.

I'd have to say...
1. I'd want nice 36c boobs
2. A curvy little waist
3. A full head of thick beautiful hair
For me:
a) a nose job
b) some hips
c) getting rid of that upper lip shadow

Eliminate the male pattern balding
20 20 vision
Washboard stomach.
A nice butt would be a close #4
Feminine voice
Slightly wider hips
Complete lack of body hair
A close #4 would have been better vision, but I think I'm cuter with my glasses on.
In my case, I'd be aiming for changes that would still allow me to switch back and forth between gender presentations. So short of a magical ability to shapeshift at will and truly change gender (complete change of body/gender at will between my male form and a smaller, actually female body) ...
1) Reduce weight to somewhere between 150 and 175 pounds.
2) Eliminate body hair and beard permanently.
3) A full head of natural hair, and no more male pattern balding or gray hair, so I could wear it in a pony tail as a male or in feminine styles as a female.

A #4 would be longer straight hair but a nice pixie cut would work just fine. Lol

1) No facial hair and finer body hair
2) Larger breasts
3) No male-pattern baldness
In fact, those are the only "defects" I have, with the possible exception of my Adam's apple... maybe a tracheal shave would be #4.
