I met with a neurologist about some numbness in my feet last week. I made sue to ask what type of exam they would be doing and how far it would go. They told me they would be testing the nerve in my lower leg, foot to knee.
Knowing this I 'knew' i could wear whatever I wanted. I opted for my new matching lace bra and panties, nice pale pink and quite sexy...
So I get there and put on the shorts they gave me.
The exam goes exactly as described; knee to foot, until the Dr. gets a funny look on her face and says, "This doesn't look right to me. We need to check further up. Lay on your stomach."
I turn over and lay down. She takes her electrode thingy and proceeds to pull my shorts down just enough to get to the lower lumbar part of my back which, of course, exposes my panties.
Without missing a beat, she pulls then out of the way with the shorts, completes testing the nerve and never says a word.
Go for broke. Maybe a bright fuchsia thong...
<Edit>Wow, Just read some of the other posts, JeanTG, and I had not honestly thought of that. I normally wear the most generic panties I have. In this case, I was absolutely certain it would never come up. Thanks for making me think. I will be sure to be more careful in the future.

Shelby That sucks Neuropathy really has no cure. I have had it for some time and it really sucks at night. It will wake me up. Doesn't much bother me in the day time but you have to watch your feet so they don't get injured blisters, etc. I spent a week with wet feet on a sailing trip and got blisters that I wasn't aware of and ended up getting treated with meds. I have been treated lots of different ways for the neuropathy and nothing really works. ONe doc wanted to give me heavy pain pills. They worked but who needs heavy pain pills, oxys in this case. I sure didn't need them and they didn't work I just didn't care about the pain. CBD oil kind of helps. Nothing seems to work well I spent lots on a chiropractor and acupuncture to no avail.
Nylons and high heels might do it but you'll have to wear them all the time LOL.

Going to the doctor for an exam is the one of the few places I don't underdress. I don't want to offend the doctor or staff. I also want them to find my problem and give me good service. I don't want to distract them.
Given that, I have had occasions where I was underdressed and had no choice or the doctor/nurse discovered my lingerie. Not much you can do in an emergency. While only once did I have any indication of an issue, but it was soon ironed out.
I have donated blood while dressed. I wore women's jeans, heeled boots, and a polyester blouse which left little question that I had on a bra over my girls. I hadn't planned on donating that day but got a call to please come in. I was too far away to go home and change to save a life.

As to underwear, 5 minutes after the photo, the Doc and a medical student had stuck a painfully large telescope up my behind, so undies would have only slowed down their examination and there's nothing SEXUAL about that procedure (not for me anyway).
Imagine a Doctor, who is offended by the naked body, how would they ever examine a boil on the butt???
The next visit, I had to be totally naked - surely that's way "more offensive" than clothed in a skirt? (like the photo but with no clothes at all, or wig, or shoes)

If you consider the mental state of a cis-guy wearing his usual male underwear going to the doctor, for him, it's pretty much a business-like approach: Get it done, move on.
If one of us is going to the doctor with the same mentality, then I would consider it authentic whichever panties are worn as long as we're prepared to own it. However, if one is viewing the appointment as an occasion, wearing special panties for it and hoping to create memories, then I would be quietly asking myself how constructive that would be.
I've never been hugely into panties, but if I were going to do this, I would wear the plainest of plain neutral-coloured panties ( black, white or 'neutral' ). No frill, no lace.
Nevertheless, it continues to be my belief that for those with the need to demonstrate more of their femme identity, a covertly feminine outer garment over plain underwear would be more expressive than flamboyantly feminine underwear under male clothing.

What a weird coincidence,, MONDAY, I have my FIRST COMPLETE FEMALE ANNUAL PHYSICAL. My SOGGF has her annual Physical also. 2 DOCTORS, A GP, an GYN/ONCOLOGIST with MAMMOGRAM complete with "Female" exam in Morning and a Dental exam in afternoon late. (see below why, my journey to becoming Tammie full time). All her Doctors except for the Dentist are in the local Hospital where SOGGF had her Breast Cancer Surgery. Its a Large regional Hospital so its going to be quite a day. Its going to be exciting to see how passable, I am to tons of people, including nurses, other "GG" Female patients and Laboratory Techs.
My SOGGF told me this last Monday, apparently she had made the appointments for BOTH of us last year with them when she made her appointments... HER Doctors are ALL Female, SOGGF tells me they all are Very Well endowed, .. SOGGF apparent told them I've been living 24/7 Fully Female, but have never had any "FEMALE" doctor exams since I start living FULLY Female about 3 years ago after SOGGF forced me to Dress FULLY En Femme FULLY in public.
When SOGGF told them this, both her GP and GYN/Oncologist, said "Well its very Important that Tammie have a FULL Female Exam and Gynecology Exam with a Mammogram, IT sounds like Tammie might want to consider Breast Implants and if so She will have to start a regimen of very strong HRT. Thus she needs a complete FEMALE exam to determine what dosage and types of hormones to prescribe for her.
What we will do is double your appointment time, We will Examine Her immediately after you then She will know what to expect, even when we "check her downstairs' like you'..... and even a mammogram (yes men can get Breast Cancer) is Mandatory. Thus I she decides ever to have reassignment surgery, we will have a baseline exam.
Any how SOGGF has decided, I will wear my Sexy Bright Orange Lacy Balconet Bra, it's Matching Retro Thong Pantie, Pantyhose, my White Sexy Pantyhose Like Deep V-Neck Bodysuit and my White Wedge 4" Strappe HiHeels

I have gone to my last couple of appointments in a skort, feels very comfortable going that way.
Rachel, there is quite a difference between walking into a doctor's office presenting as a female and going into an examination presenting as male in anticipation of a panty reveal. Your presentation disarms the staff and in the event they perceive you as trans-whatever rather than female, they can make fair assumptions going into the exam about you as a person. Even better if they know you as a regular patient, they might have an understanding that you are transitioning or a crossdresser or somewhere in between. Bottom line, I don't think anyone here is questioning whether someone who presents much/most/all of the time as a female for whatever reason should present as such to their physician unless there are less than sincere reasons for doing so.
As for Tammie, your story makes my head spin. Completely uncertain about what was just added to this conversation but in all honesty, it reads like fantasy fiction.

Yes but you are presenting female. It is reasonable for a doctor that doesn't know you, say a specialist, to simply assume you are in transition and this is your *normal* presentation. I mentioned in my reply "unless you are in transition". I don't know what your relationship is with your GP and if (s)he knows you are TG or not.
My point was in response to the OP: "what panties should I wear to my doctor visit". I may be reading this wrong but it sounds like there's a degree of *wanting* to show the doctor you wear panties. This, IMHO, is what is wrong, and what I can guarantee, a female physician in particular, doesn't like. Obviously in an emergency you are wearing what you are wearing, but if you have a doctor's appointment planned in advance, please wear drab underwear, or if you don't have any, something plain in a neutral colour like black, grey or white.
Again, if you are transitioning, or you live your life presenting as female, it's a different matter.
Also, don't make the assumption that if your doctor's visit is to check your throat (s)he wont want to look at other things, as happened with the neurologist above. Don't be surprised at a "when was the last time you had your prostate checked"? I am diabetic and one day it was "today I'm going to check your feet as part of diabetic foot care". Good thing I had taken off my toenail polish...
Always keep in mind that doctors, while professionals, are also human beings. And women, whether a doctor or not, see things differently than men and are *very* sensitive to any hints of sexual inappropriateness in the examining room. We may not think that our proclivity to wear women's underwear is sexual, but the doctor may not be all that aware of trans matters. They typically don't see that many of us unless in a practice oriented towards transgender care! For may doctors, especially small-town GPs, transgenders are a curiosity in a textbook that they may see once or twice in their practice over their whole career.
Honestly, I'm with you Sara, I call BS on that story. It also makes no sense from a medical standpoint. The last thing an endocrinologist will check before prescribing HRT is what is down below, other than maybe doing a cursory all-over physical. It's the lab work that the endo will be most interested in. And a gynecologist is not where a TS goes for HRT. That is simply not their bailiwick and a pre-op TS most certainly does not need a "FULL FEMALE EXAM", she needs blood work to determine testosterone levels to prescribe blockers, and liver function tests and other endo tests to rule out contraindications for HRT. About the only interaction a gynecologist may have with someone TS, is someone already on HRT who needs a mammogram, which TS women with breast development should get.

I must admit that procedure does wipe the smile off your face , I had to strip and wear a surgical gown anyway . While I won't go into details the examination does leave the bowels a little loose , so it's best not to wear your Sunday best .

I've donated blood three times now dressed , the only problem the first time was I was wearing a skirt , ( in fact the only one of twenty in the waiting area ) and the screening nurse called my full male name out , yes it did raise a few eyebrows . I've now officially changed it to the shortend form so it's female combatible .
